U.S. Transportation Department Office of Inspector General to Audit FAA Pilot Training Requirements

WASHINGTON (Reuters) – The U.S. Transportation Department’s Office of Inspector General said on Monday it will audit Federal Aviation Administration pilot training requirements for U.S. and foreign air carriers after two deadly crashes of Boeing’s <BA> 737 MAX.

The audit will also review international civil aviation authorities’ requirements for carriers’ pilot training regarding the use of flight deck automation.

Pilots have been harshly critical of Boeing’s decision not to disclose details of a new automation system – known as the Maneuvering Characteristics Augmentation System, or MCAS – that has been linked to both fatal crashes.

The Inspector General cited a report by Indonesia’s Lion Air that “responses to erroneous activations of MCAS contributed to the crash, raising international concerns about the role of pilot training.”

The report said Boeing’s safety assessment assumed pilots would respond within three seconds of a system malfunction. But on the fatal flight and one that experienced the same problem the previous evening, it took both crews about eight seconds to respond.

Boeing declined to comment on the new review.

The FAA said it would cooperate with the inspector general’s review. “Raising and harmonizing pilot training standards across the globe are among the FAA’s top aviation safety priorities,” the FAA said. “We continue to pursue expanded conversations among the world’s aviation regulators to identify ways to enhance international aviation safety through robust pilot training programs.”

Boeing has proposed new simulator training for pilots on a series of scenarios before they are allowed to resume 737 MAX flights.

The MAX is not expected to be freed to fly until late April at the earliest. In March, the department’s IG said it would audit the FAA’s certification of the Boeing 737 MAX.

The Trump administration on Monday proposed an additional $30 million in it 2021 budget “to improve aviation oversight, following recommendations from the Boeing 737 MAX investigations.”

The funding would support 13 new full-time positions for the creation of an office mandated by Congress to oversee the FAA’s delegation of some certification tasks to Boeing and other plane-makers. The FAA would also use some of the funds for data collection and for “technological advances that we use to assess safety data,” Deputy FAA Administrator Dan Elwell said.

United Buys Arizona Flight Academy to Feed Pilot Pipeline

CHICAGO, Feb 5 (Reuters) – United Airlines Holdings Inc announced on Wednesday an agreement to purchase a flight training academy in Phoenix in a move aimed at bolstering its pilot pipeline as the industry faces a global shortage.

To address a tight U.S. labor market created by years of slow pilot hiring, a wave of pending retirements and new rules that in 2013 increased the number of required training hours, U.S. airlines have been taking steps to attract young aviators.

Chicago-based United, which is looking to hire more than 10,000 pilots by 2029, will be the first major U.S. carrier to run its own academy.

“We think this program will alleviate any shortage we would have had and that’s its purpose,” Curtis Brunjes, United’s managing director of pilot strategy, told reporters.

The school, currently operating as Westwind School of Aeronautics, will be renamed United Aviate Academy in September.

United expects approximately 300 graduates in 2021 and wants to expand capacity to accommodate 500 graduates per year, Brunjes said, noting that the academy is among the airline’s most aggressive steps on pilot hiring since the 1960s.

One area of focus at the school will be training for loss of control incidents, a leading cause of plane disasters, that goes beyond the current U.S. Federal Aviation Administration requirements.

Some of the enrollees will come from Aviate, a recruitment program that United launched last year offering students and pilots from 15 schools and regional carriers a path to a job at the major.

American Airlines Group Inc and Delta Air lines Inc have similar career programs.

United – which plans to design the academy’s curriculum in concert with the Air Line Pilots Association, International – is offering financing options for training and will also launch a scholarship program focused on women and minorities.

It did not disclose financial details of the purchase, with Brunjes saying only that the company paid “more than asset value, but not hugely more.”

The financial benefit down the line will be ensuring that regional carriers, which operate a significant amount of U.S. airlines’ domestic capacity at a lower cost, have enough pilots to fly the routes that United wants to contract.

In recent years, regional carriers have had to double salaries and offer sign-on bonuses to attract pilots to the field, driving up costs.

Boeing Co expects 800,000 new pilots will be needed over the next 20 years to meet growing demand for air travel.

Air-to-Air Refueling Broadens C295 Tactical Airlifter’s Operational Versatility

The new capability for Airbus’ C295 to serve as an aerial tanker brings additional flexibility for this tactical airlifter, further increasing its already-proven versatility and opening additional mission opportunities.

Airbus Defence and Space developed a removable air-to-air refuelling (AAR) kit for the twin-engine aircraft that utilises a 100-ft.-long deployable hose with a “basket” at the end – enabling the transfer of fuel to receiver aircraft equipped with a probe. A remote vision system allows crewmembers aboard the C295 tanker to monitor refuelling operations from the cargo cabin.

Recent flights to test the kit involved an Airbus-owned C295 that refuelled a C295 from the Spanish Air Force, as well as proximity tests with the C295 and a fast fighter aircraft – a Spanish Air Force F-18.

“Customers are increasingly interested in purchasing not just an aircraft, but a versatile platform that can be used for different missions,” explained Martín Espinosa, the Airbus Defence and Space engineering technical manager responsible for the C295’s aerial refuelling test campaign. “The development of the air-to-air refuelling capabilities of the C295 forms part of this strategic vision.”

The C295’s aerial refuelling capability would be a highly valuable mission-extender for customers using C295s. These customers include the armed forces of current and future C295 operators responsible for civil and military search and rescue missions.

Additionally, it could serve as a cost-effective platform to train fighter pilots in the skills needed for air-to-air refuelling. “The C295 tanker kit could facilitate training of fighter pilots for missions involving refuelling, or even for AAR services on a lease-by-the-hour basis at a fraction of the cost of heavier aircraft,” explained Luis Díaz-Miguel, the Tactical Airlifters Marketing Manager.

Captain Gabiña, a Spanish Air Force pilot involved in the aerial refuelling tests, gave high marks to the C295 in its new role as tanker. “The degree of difficulty in flight test is always high since it involves performing manoeuvers that no one has done before. It should be noted that due to the positive behaviour of the aircraft, the operation has been good and straightforward,” he said.

Air Canada Boeing 767 Makes Emergency Landing in Madrid

An Air Canada Boeing 767 aircraft made a safe emergency landing at Madrid’s Adolfo Suarez-Barajas International Airport after part of the jets landing gear reportedly fell off and entered its engines.

The Toronto-bound flight touched down shortly after 7.10pm Monday evening with fire engines lining the runway.

Kobe Bryant Dies in California Helicopter Crash

The sports world was in mourning following news Sunday that Kobe Bryant, his daughter Gianna, and seven other people died died in a helicopter crash. Weather is believed to have been a contributing factor in the accident, as the conditions at the time were poor. The National Transportation Safety Board has sent a team of investigators to the site.

The helicopter reportedly departed John Wayne Airport in Orange County at 9:06 a.m. local time. The group was heading to the Mamba Sports Academy in Newbury Park. The Federal Aviation Administration reported that no flight plan had been filed. The Sikorsky S-76B helicopter was flying in low clouds and fog, and was operating under “special VFR” (special visual flight rules) status, requiring the pilot to be responsible for determining safe flying requirements in challenging weather conditions..

The helicopter crashed in foggy conditions on steep terrain in the mountains near Las Virgenes Road in Calabasas, California. The crash was reported to the Los Angeles County Fire Department. The crash set off a small brush fire in the area. Firefighters were able to contain the blaze, but were not able to find any survivors.

Remains of 6 Recovered from Hawaii Helicopter Crash

  • No sign of any survivors

(Reuters) – Teams combing the wreckage of a Hawaii sightseeing helicopter that crashed on Kauai island found no sign of survivors on Friday and recovered six sets of human remains before suspending the search due to bad weather, police and fire officials said.

The grim announcement came in a news conference almost 24 hours after the aircraft, first reported missing on Thursday evening, went down in a remote area of rugged terrain near the end of a tour flight over the island’s famed Na Pali Coast. 

The crash was at least the ninth, and by far the deadliest, involving sightseeing helicopters in Hawaii over the past five years, according to National Transportation Safety Board (NTSB) records. 

The confirmed manifest of the ill-fated aircraft, flown by Kauai-based tour operator Safari Helicopters, consisted of six passengers, two of them children, and one pilot, Kauai County fire battalion chief Solomon Kanoho told reporters. 

The identities of the dead were being kept confidential until next of kin could be notified, authorities said. 

“We are heartbroken by this tragedy and we continue to ask the public to consider the sensitive nature of this devastating situation,” Mayor Derek Kawakami said in a statement. “Our thoughts and prayers are with the families and loved ones of all victims during this extremely difficult time.” 

The Kauai fire department called off its search-and-recovery efforts late Friday afternoon due to fog and poor visibility but planned to resume the operation at daybreak on Saturday, Kanoho said. 

Although the remains of just six of the seven people who were aboard the ill-fated aircraft have been recovered, Kanoho added: “There are no indications of survivors.”

TOURISTS FROM TWO FAMILIES 

Kanoho previously said the passengers on board the helicopter had been in two groups – a party of two from one family and a party of four from another. 

Kanoho declined to describe details of the wreckage out of respect for the victims’ loved ones. 

While the cause of the crash has yet to be determined, Kanoho said the area where the helicopter went down had experienced “some very bad weather” beforehand, adding that the chopper had crashed within its prescribed flight route. 

The NTSB, which said it was sending a three-member team to investigate the crash, reported in May that there had been eight accidents involving Hawaii tour helicopters over the past five years, with four deaths and 18 injuries. 

The agency made that report after a tour helicopter went down in a residential neighborhood on the island of Oahu in April, killing three people. 

The latest crash was in Koke’e State Park in an area called Nu’alolo, a steep-sided valley north of Waimea Canyon State Park, according to a statement posted by the Kauai police department on Facebook. 

Waimea Canyon is a tourist destination known as the “Grand Canyon of the Pacific,” and police said the helicopter was last heard from at about 4:40 p.m. on Thursday, when the pilot radioed that the aircraft was just departing that area. 

A search was launched a short time later, after Safari alerted authorities that the helicopter was 30 minutes overdue on its flight back to the airfield in Lihue on the island’s southeast end, officials said. 

A U.S. Coast Guard cutter vessel and helicopter search crew were immediately dispatched. The search was expanded at daybreak on Friday to include air, sea and ground teams from the Coast Guard, U.S. Navy, police, fire department and other agencies. 

The missing aircraft was equipped with an electronic locator beacon, but no signals were received after it disappeared, the Coast Guard said. 

According to its website, Safari offers aerial sightseeing excursions to Kauai’s major attractions over the Na Pali Coast and Waimea Canyon. The Na Pali Coast, known for jagged green cliffs laced with towering waterfalls, is one of the most visited attractions on Kauai, the fourth-largest island in the Hawaiian chain.

FAA Must Boost Oversight to Address Allegiant Air Maintenance Issues

WASHINGTON (Reuters) – The U.S. Federal Aviation Administration (FAA) needs to improve its oversight to address maintenance issues at Allegiant Air, the 11th largest U.S. carrier, according to a report seen by Reuters on Tuesday.

The U.S. Transportation Department’s Inspector General, in a 31-page report sent to Capitol Hill on Tuesday but not yet made public, said FAA inspectors since 2011 have not “consistently documented risks associated with 36 Allegiant Air in-flight engine shutdowns for its MD-80 fleet or correctly assessed the root cause of maintenance issues.”

Ultra-low cost Allegiant, a unit of Allegiant Travel Co, said it had not yet see the report and did not have an immediate comment.

The FAA said in a letter attached to the report that it agreed with eight of the nine recommendations made by the inspector general and partially agreed with the remaining one.

Allegiant carried about 14 million passengers last year, serving 122 U.S. cities and Puerto Rico on 450 flight routes.

The inspector general opened the probe in May 2018 after a “series of in-flight engine shutdowns, aborted takeoffs, and unscheduled landings” raised concerns about maintenance practices.

The report said in-flight shutdowns at Allegiant “continued until July 2018 and were only resolved four months later when Allegiant Air retired the last of its MD-80 fleet.” Allegiant now flies an all Airbus fleet.

The report found in-flight engine shutdowns forced 21 Allegiant aircraft to return or divert to other airports between 2014 and 2018, but that regulators did not properly track engine shutdown risks.

A 2015 maintenance provider failure at Allegiant Air demonstrated “severe violations that represent unacceptable safety risks or could result in catastrophic outcomes should also warrant a more stringent oversight approach,” the report said.

The inspector general said the airline’s maintenance provider failed to insert a cotter pin on a critical flight control component that put some 30,000 passengers at risk.

The report said in August 2015, a pilot “almost lost control of this aircraft during takeoff when it unexpectedly tried to lift off prematurely” but was able to abort takeoff and land safely.

After inspectors proposed a 30-day suspension for Allegiant Air’s maintenance provider, FAA regional officials reduced the suspension to a compliance action. FAA inspectors closed out six of eight compliance actions before ensuring Allegiant Air actually took any corrective actions, the report found.

It also found that FAA does not provide inspectors with guidance and comprehensive training to ensure Allegiant Air takes appropriate corrective actions.

The FAA said it had “initiated compliance actions at Allegiant Air that have improved safety for the flying public.”

ATSB Australia Investigating Separation Incident at Albury Airport

The Australian Transport Safety Bureau (ATSB) says it is investigating a separation issue involving a Virgin Australia ATR 72-600 turboprop and Piper PA-28 light aircraft at Albury Airport.

The incident occurred on October 19 2019 when the Virgin Australia 72-600 VH-FVR was operating a regular public transport (RPT) flight from Sydney to Albury.

While passing through 1,300 ft and on a straight in approach to runway 25 at Albury Airport in visual meteorological conditions, the ATSB said the Virgin Australia turboprop received a traffic collision avoidance system alert on the PA-28.

The single-engine PA-28 VH-XDI, which was operated by the Australian Airline Pilot Academy, was turning final for Runway 25, the ATSB said in a short statement.

“The flight crew of the ATR 72 conducted a missed approach to increase separation between the two aircraft,” the ATSB said.

“As part of the investigation, the ATSB will interview directly involved parties and obtain other relevant information, including recorded data.”

The ATSB described the event as a near collision and a serious incident. There were no injuries.

Further, it said the investigation was expected to be completed by the second quarter of calendar 2020.

However, should a critical safety issue be identified during the course of the investigation, the ATSB said it would immediately notify relevant stakeholders in order that appropriate and timely safety action could be taken.

The Australian Airline Pilot Academy is a subsidiary of Regional Express (Rex), which recently bought another pilot training school based in Ballarat, ST Aerospace Academy.

Virgin Australia had eight ATR 72 turboprops in its fleet.

General Dynamics Begins Gulfstream G500 Deliveries to Europe
