U.S. Transportation Department Office of Inspector General to Audit FAA Pilot Training Requirements

WASHINGTON (Reuters) – The U.S. Transportation Department’s Office of Inspector General said on Monday it will audit Federal Aviation Administration pilot training requirements for U.S. and foreign air carriers after two deadly crashes of Boeing’s <BA> 737 MAX.

The audit will also review international civil aviation authorities’ requirements for carriers’ pilot training regarding the use of flight deck automation.

Pilots have been harshly critical of Boeing’s decision not to disclose details of a new automation system – known as the Maneuvering Characteristics Augmentation System, or MCAS – that has been linked to both fatal crashes.

The Inspector General cited a report by Indonesia’s Lion Air that “responses to erroneous activations of MCAS contributed to the crash, raising international concerns about the role of pilot training.”

The report said Boeing’s safety assessment assumed pilots would respond within three seconds of a system malfunction. But on the fatal flight and one that experienced the same problem the previous evening, it took both crews about eight seconds to respond.

Boeing declined to comment on the new review.

The FAA said it would cooperate with the inspector general’s review. “Raising and harmonizing pilot training standards across the globe are among the FAA’s top aviation safety priorities,” the FAA said. “We continue to pursue expanded conversations among the world’s aviation regulators to identify ways to enhance international aviation safety through robust pilot training programs.”

Boeing has proposed new simulator training for pilots on a series of scenarios before they are allowed to resume 737 MAX flights.

The MAX is not expected to be freed to fly until late April at the earliest. In March, the department’s IG said it would audit the FAA’s certification of the Boeing 737 MAX.

The Trump administration on Monday proposed an additional $30 million in it 2021 budget “to improve aviation oversight, following recommendations from the Boeing 737 MAX investigations.”

The funding would support 13 new full-time positions for the creation of an office mandated by Congress to oversee the FAA’s delegation of some certification tasks to Boeing and other plane-makers. The FAA would also use some of the funds for data collection and for “technological advances that we use to assess safety data,” Deputy FAA Administrator Dan Elwell said.

Boeing to Give Southwest Board 737 MAX Update This Week

CHICAGO (Reuters) – Boeing Co <BA> this week will present to the board of its largest 737 MAX customer, Southwest Airlines Co <LUV>, an overview of its plans to return the grounded jet to service, a spokesman for the airline said on Monday.

The meeting on Wednesday and Thursday comes after Southwest Chief Executive Gary Kelly said last month that the airline could look next year at diversifying its fleet beyond Boeing 737 aircraft. Budget-friendly Southwest has structured its business model around flying only 737 aircraft for the past 50 years and bet its entire growth strategy on the 737 MAX, the latest iteration of Boeing’s narrowbody workhorse.

With the MAX parked since mid-March following crashes on Lion Air and Ethiopian Airlines that together killed 346 people, Southwest has had to scale back its growth plans and cancel north of 100 daily flights, wiping $435 million from its earnings between January and September.

Kelly, who is also Southwest’s chairman of the board, invited Boeing to address the timing and logistics of dozens of 737 MAX deliveries that it was supposed to receive this year. The meeting will also give Boeing a chance to defend its product and the steps it is taking to restore public confidence after the two fatal crashes, sources said.

“It’s an overview of the Return to Service Plan, timing, and plans moving forward,” Southwest spokesman Chris Mainz said. “Just a good chance for our Board to hear directly from Boeing, but nothing more to it than that.”

It is not the first time that Boeing has presented to a regularly scheduled board meeting, he said.

Southwest had 34 MAX jets in its fleet when global regulators grounded the aircraft in March. The airline was supposed to receive 41 more 737 MAX planes before the end of the year, but most of those deliveries are now scheduled for 2020.

Hundreds of undelivered 737 MAX jets are parked at Boeing facilities in Washington state, where the planemaker is facing a delivery logjam once the U.S. Federal Aviation Administration gives approval for them to fly commercially.

While Boeing is targeting approval in December, the FAA has pushed back on any fixed timeline.

Southwest has removed the 737 MAX from its flying schedule until early March. The airline has said it will need one to two months to train its pilots and prepare the jets for flight once regulators approve new software and pilot training.

Unraveling The Boeing 737 MAX Lion Air Crash

(Reuters) – The crash of a Boeing Co 737 MAX jet in Indonesia on Oct. 29 has raised questions on whether the manufacturer shared enough information with regulators, airlines and pilots about the systems on the latest version of its popular narrow-body plane.

The jet operated by budget carrier Lion Air crashed into the Java Sea shortly after take-off from Jakarta, killing all 189 people on board.

WHAT IS NEW ON THE 737 MAX?

The most hyped features of the 737 MAX compared with its predecessor, the 737NG, are more fuel-efficient engines.

But as a result of the larger engines, which are placed higher and further forward of the wing, the jet’s balance changed. To address that, Boeing put in place more anti-stall protections, Leeham Co analyst Bjorn Fehrm said in an online post.

An automated protection system called the Maneuvering Characteristics Augmentation System (MCAS) kicks in when the angle of attack is too high, when the plane’s nose is too elevated, threatening a stall.

WHAT IS ‘ANGLE OF ATTACK’?

On paper, it measures the angle between the air flow and the wing. But it is so fundamental to flight that historians say the only instrument on the Wright Brothers’ first aircraft was a piece of yarn designed to measure it.

If the angle of attack is too high, the airflow over the wing is disturbed, throwing the plane into an aerodynamic stall.

One of two angle of attack sensors on the Lion Air jet was faulty, according to Indonesian investigators.

The U.S. Federal Aviation Administration (FAA) last week warned airlines that erroneous inputs from those sensors could lead the jet automatically to pitch its nose down even when autopilot is turned off, making it difficult for pilots to control.

WHICH AIRLINES OPERATE THE 737 MAX?

Boeing has delivered 241 of the jets to customers since it entered service last year, according to its website.

Major operators include Southwest Airlines, American Airlines, Norwegian, Lion Air, Air Canada, China Southern, China Eastern and flydubai.

Another 4,542 have been ordered but not yet delivered.

WHAT DID AIRLINES AND PILOTS KNOW ABOUT THE SYSTEM?

Lion Air’s flight manual did not contain information about the new anti-stall system, according to investigators and an airplane flight manual seen by Reuters. U.S. pilots were also not made aware in training courses, pilot unions say.

Boeing Chief Executive Dennis Muilenburg told Fox Business Network on Tuesday that Boeing provides “all of the information that’s needed to safely fly our airplanes”.

HOW WOULD A PILOT SHUT OFF THE SYSTEM?

Pilots can stop the automated response by pressing two buttons if the system behaves unexpectedly, the FAA says.

That action is set out in a checklist used by Lion Air pilots for in-air troubleshooting, an instructor said. It is also required to be committed to memory by pilots.

Pilots on a flight from Jakarta to Bali the day before the crash experienced a similar sensor issue but managed to land safely by turning off the system, the New York Times reported.

HOW WAS THE SYSTEM APPROVED?

The FAA holds the main responsibility for certifying Boeing jets and training programs for pilots, but local regulators also issue approvals for airlines based in their countries.

An unresolved question is how Boeing measured the system’s reliability and on what basis the FAA certified it as safe.

HOW ARE PILOTS TRAINED?

An FAA document on training requirements for 737 MAX pilots transitioning from the older 737NG has no reference to the new anti-stall system.

Lion Air says it followed a training regime approved by U.S. and European regulators. The training was restricted to three hours of computer-based training and a familiarization flight.

However, Brazil’s regulator told Reuters that it had required specific training for pilots on the anti-stall system.

WHAT HAS CHANGED SINCE THE CRASH?

Boeing last week issued a bulletin to airlines reiterating existing procedures and advising them to add information on the anti-stall system to flight manuals, which was quickly followed by an FAA directive making that mandatory.

The FAA and Boeing are studying the need for software changes, as well as revisions to training and operating procedures on the 737 MAX, the regulator said.

WHEN WILL THE FIRST REPORT ON THE CRASH BE RELEASED?

A preliminary report will be released on Nov. 28 or 29, according to Indonesian investigators. However, divers have yet to locate the airline’s cockpit voice recorder, which would shed light on pilot interactions that are important for gaining a fuller picture of the circumstances of the crash.

US Safety Board Hearing On Southwest Engine Explosion

Nov 14 (Reuters) – The U.S. National Transportation Safety Board is set to hold an investigative hearing on Wednesday about a midair incident in April during which an engine on a Southwest Airlines Boeing 737 exploded over Pennsylvania, killing one passenger.

Dallas-based Southwest has been under intense scrutiny in the months since an engine on a flight headed from New York to Dallas blew apart, shattering a plane window, flinging shrapnel and killing passenger Jennifer Riordan, one of 149 people aboard.

The episode, which has raised concerns about the safety of similar engines, was the first fatality on a U.S. commercial passenger airline since 2009.

The all-day hearing in Washington will focus on the fan blade design and development history of the engine type that failed, a CFM56-7B made by CFM International, a transatlantic joint-venture between General Electric Co and France’s Safran SA, the NTSB said.

The hearing will also focus on engine fan blade inspection methods and engine fan blade containment design and certification criteria, the NTSB said.

Representatives from Chicago-based planemaker Boeing, CFM, and Southwest Airlines are due at the hearing.

The companies did not immediately respond to requests for comment.

The hearing comes as Indonesian authorities investigate last month’s deadly Lion Air crash involving a newer version of Boeing’s best-selling single-aisle aircraft, the 737 MAX.

Boeing Issues Advice For Pilots After Indonesia Crash

ZHUHAI, China/WASHINGTON (Reuters) – Boeing Co (BA.N) said on Wednesday it had issued a safety bulletin reminding pilots how to handle erroneous data from a key sensor in the wake of last week’s Indonesian jetliner crash.

The U.S. planemaker said investigators probing the Lion Air crash off the coast of Indonesia, in which all 189 on board were killed, had found that one of the “angle of attack” sensors on the brand-new Boeing 737 MAX jet had provided erroneous data.

Experts say the angle of attack is a crucial parameter that helps the aircraft’s computers understand whether its nose is too high relative to the current of air – a phenomenon that can throw the plane into an aerodynamic stall and make it fall.

Some modern aircraft have systems designed to correct the posture of the aircraft automatically to keep flying safely.

There are also procedures for pilots to follow in the event of missing data from damaged sensors on the fuselage, but it remains unclear how much time the crew of flight JT610 had to respond at the relatively low altitude of around 5,000 feet.

An angle of attack sensor had been changed by mechanics on the ground in Bali the day before the crash, Indonesia’s National Transportation Safety Committee (KNKT) said.

The captain and first officer flying from Bali to Jakarta the night before the crash had indicators displaying differences in angle of 20 degrees, KNKT said, but that flight landed safely despite the issues in the air.

COCKPIT PROCEDURE

Boeing said in a statement received at China’s largest air show in Zhuhai that its note to airlines underscored “existing flight crew procedures” designed to address circumstances where information coming into the cockpit from the sensors was wrong.

The Boeing 737 MAX has three such blade-shaped sensors. Erroneous readings can in some circumstances cause the 737 MAX to point the nose down sharply to keep air under the wings and avoid a stall, according to a person briefed on the matter.

A source said on condition of anonymity that the Boeing bulletin related only to the 737 MAX, of which there are just over 200 in service.

Service bulletins can be followed by mandatory airworthiness directives by the U.S. Federal Aviation Administration.

Boeing has delivered 219 737 MAX jets to customers globally, with 4,564 orders for jets yet to be delivered.

The Boeing 737 MAX is a more fuel-efficient version of the manufacturer’s best-selling single-aisle 737 series.

The Lion Air crash was the first involving the new version, which airlines introduced into service last year.

Indonesian authorities have downloaded information from the flight data recorder that showed a cockpit indicator on the Lion Air jet was damaged for its last four flights.

A search for the cockpit voice recorder, the second so-called “black box”, remains underway.

KNKT said it would attempt to reconstruct the jet’s last flight using Boeing simulators in Seattle. The angle of attack sensor replaced in Bali would be analysed at its place of manufacture in Chicago, the accident investigator said.

Indonesian Searchers Find Lion Air Black Box

JAKARTA, Nov 1 (Reuters) – Indonesian authorities on Thursday retrieved a flight data recorder from a Lion Air jet that crashed and broke apart in shallow sea near the capital, Jakarta, this week, killing all 189 people on board.

The country’s second-deadliest air disaster since 1997 has prompted renewed concern about Indonesia’s patchy aviation safety record, and the government has said Lion Air will face tougher safety regulation.

Investigations into the world’s first crash of a Boeing Co 737 MAX, introduced into commercial service last year, will be scrutinised by the global aviation industry.

“Hopefully, this can unveil the mystery behind the plane crash,” Indonesia’s transportation safety committee chief Soerjanto Tjahjono told a news conference at Jakarta’s main port after receiving the device, known as a black box.

The data it holds should provide clues to what went wrong with the plane, which had only been in service since August.

It lost contact with ground staff just 13 minutes after taking off early on Monday from Jakarta, on its way to the tin-mining town of Pangkal Pinang.

The pilot had asked to return to base shortly after take-off, and ground control officials had approved the request.

A navy diver told broadcaster Metro TV on board a search vessel his team found the orange-coloured box intact in debris on the muddy sea floor.

Indonesia’s transportation safety committee (KNKT) will analyse its data in Jakarta, which could take up to two weeks.

Searchers have yet to find the second black box containing recordings of cockpit conversations. Strong currents have hampered search efforts, complicated by the presence of energy pipelines in the area.

The discovery of the black box may provide some relief to grieving relatives. But hopes are fading of finding a large section of fuselage intact with bodies, easily retrievable, inside.

The commander of the navy divers involved in the search was quoted by the Kompas.com news portal as saying divers had found many bodies. But only one has been identified.

“What is important for us is to get more information about the victims because having their remains back is important for us so we can bury them properly,” said Ade Inyo, whose brother in law was on the flight.

MORE INSPECTIONS, SAFETY REVIEW

The investigation will be carried out with help from Boeing, General Electric and the Federal Aviation Federation, officials have said.

It will also focus on four of Lion Air’s staff including its technical director who were suspended by Indonesia’s transportation ministry on Wednesday amid speculation the aircraft was not airworthy.

“For now, we will focus on two primary causes,” KNKT deputy chief Haryo Satmiko told Reuters, referring to equipment and the people who flew, maintained and managed the aircraft.

The transport ministry suspended for 120 days Lion Air’s maintenance and engineering director, fleet maintenance manager and the release engineer who gave the jet permission to fly on Monday, it said in a press release.

Founded in 1999, the privately owned budget carrier’s aircraft have been involved in at least 15 safety incidents and it has faced tougher international safety restrictions than other Indonesian airlines.

It will now be subjected to more intensive “on ramp” inspections compared with other airlines, authorities said.

President Joko Widodo has also ordered a review of all regulations relating to flight safety.

Indonesia is one of the world’s fastest-growing aviation markets. Its transportation safety committee investigated 137 serious aviation incidents from 2012 to 2017.

Lion Air said the aircraft that crashed had been airworthy and the pilot and co-pilot had 11,000 hours of flying time between them.

But according to the transport safety committee, the plane had technical problems on its previous flight on Sunday, from the city of Denpasar on the resort island of Bali, including an issue over “unreliable airspeed”.

Lion Air chief executive Edward Sirait has acknowledged reports of technical problems with the aircraft, but said maintenance had been carried out “according to procedure” before it was cleared to fly again.

Lion Air’s only other fatal accident was in 2004, when an MD-82 crashed upon landing at Solo City, killing 25 of the 163 people on board, according to the Flight Safety Foundation’s Aviation Safety Network.

In April, the airline announced a firm order to buy 50 Boeing 737 MAX 10 narrowbody jets with a list price of $6.24 billion. It is one of the U.S. planemaker’s largest customers globally, and was the first carrier globally to take delivery of the 737 MAX last year.

Lion Air 737 Crashes In Java Sea With 188 Passengers Aboard

JAKARTA (Reuters) – A Lion Air flight with at least 188 people on board is believed to have sunk after crashing into the sea off Indonesia’s island of Java on Monday, shortly after take off from the capital on its way to the country’s tin-mining hub, officials said.

A spokesman for Indonesia’s search and rescue agency said the Lion Air flight, JT610, lost contact 13 minutes after takeoff, adding that a tug boat leaving the capital’s port had seen the craft falling.

“It has been confirmed that it has crashed,” the spokesman, Yusuf Latif, said by text message, when asked about the fate of the Lion Air plane, which air tracking service Flightradar 24 identified as a Boeing 737 MAX 8.

Debris thought to be from the plane, including aircraft seats, was found near an offshore refining facility, an official of state energy firm Pertamina said.

Wreckage had been found near where the Lion Air plane lost contact with air traffic officials on the ground, said Muhmmad Syaugi, the head of the search and rescue agency.

“We don’t know yet whether there are any survivors,” Syaugi told a news conference. “We hope, we pray, but we cannot confirm.”

Flight JT610 took off around 6.20 a.m. and was due to have landed in the capital of the Bangka-Belitung tin mining region at 7.20 a.m., the Flightradar 24 website showed.

“We cannot give any comment at this moment,” Edward Sirait, chief executive of Lion Air Group, told Reuters, adding that a news conference was planned for later on Monday. “We are trying to collect all the information and data.”

Preliminary flight tracking data from Flightradar24 shows the aircraft climbed to around 5,000 feet (1,524 m) before losing, and then regaining, height, before finally falling towards the sea.

It was last recorded at 3,650 feet (1,113 m) and its speed had risen to 345 knots, according to raw data captured by the respected tracking website, which could not immediately be confirmed.

Its last recorded position was about 15 km (9 miles) north of the Indonesian coastline, according to a Google Maps reference of the last coordinates reported by Flightradar24.

The accident is the first to be reported that involves the widely-sold Boeing 737 MAX, an updated, more fuel-efficient version of the manufacturer’s workhorse single-aisle jet. The first Boeing 737 MAX jets were introduced into service in 2017.

Lion Air’s Malaysian subsidiary, Malindo Air, received the very first global delivery.

Boeing is aware of the airplane accident reports and is “closely monitoring” the situation, it said on social network Twitter.
